At-home chronic condition management, powered by AI

Chronic conditions (obesity, type 2 diabetes, cardiovascular disease) affect millions of people, yet most healthcare systems still treat them reactively and in-clinic. eMed has spent years proving that the opposite is possible: that patients managed at home, with the right tools, can achieve outcomes far beyond what traditional care delivers — 90% adherence rates where the industry averages 40–60%. We're not looking for incremental improvements to existing apps. We want genuinely novel ideas: new ways to support patients through long programmes, surfaces for monitoring and coaching that feel human, tools that make adherence effortless, or entirely new ways to think about how AI can sit alongside someone managing a long-term health condition.
